Haven't seen it discussed, does Classic Shell

work with Win7 starter edition?
I don't have an answer to that specific question, but it may not be
as necessary as you think. For instance, one feature is bringing
back the "Recent Documents" menu. But that's actually not needed.
Click the Start button and start typing the file name or a recently-
used document; Windows finds it for you.

I was looking for something to change the desktop appearance, which you
can't do with starter.

You can, somewhat. I found a utility that allows you to change the
desktop wallpaper on Win7 Starter. It's not as convenient as it could
be, since it forces you to reboot for the new wallpaper to take effect,
but it's better than having no control at all. If you can't find the
utility I'll see if I can find out where I got it from.

Irfanview can do it in Home Premium, and no reboot is required. (I
just now tried it.) I'd be interested to hear whether it can do it in
Starter also.

Irfanview is a free image-viewing and -editing utility that can do
everything up to and including red-eye correction in photos. I use
it quite often, and have been happy enough with it that I sent a
voluntary contribution.

I'm a big fan of Irfanview, but it's not able to change the desktop
wallpaper in Windows 7 Starter.
