Got cdd.dll Bsod on exiting from a game to desktop

Hello Guys please help me .. Sometimes when i try to exit from any game to my desktop i got cdd.dll blue screen error.

My pc config is

processor = AMD fx 6300 six core
Motherboard = Asus M5A78LM-usb 3
Ram = 2+4 gb
HDD = 1 tb
GPU = zotac nvidia gtx 750 ddr5 2gb


And i also attached dump file plz help me guys

You didn't attach any dump files. All you attached was Seven Forums data collection tool.
