Hmm. Reading the help page suggests that that form will actually pop up
from within the software itself. If for some strange reason they really
have designed it so that it only pops up the first time you use the
software, then uninstalling it and reinstalling it _might_ help - but I
doubt they would have done that.

If by "the Help File" you mean that those are the options below "Help"
in the menu (I was forgetting that Microsoft stopped the across-window
menus appearing by default, from Vista onwards - I'm still on XP here),
then I can't help any further, other than to suggest the uninstall and
reinstall route. If that _isn't_ what you meant, then I'd try turning on
menus (I forget how one does that in Vista/7), and see if there _is_
anything under Help. (When I say menus, I mean the row of words, usually
starting with "File Edit View" and ending with "Tools [or Options]
Help", that appear across the top of most applications.) There's often
either a Registration option, or something (such as a button or link)
inside the "About" popup. (I'm _not_ talking about "About" in the help
_system_.)

Perhaps if you post somewhere a screenshot of what the application looks
like when you launch it? [Ideally with "Help | About" popped up over
it.]

They can't send you the real form because the real form is in the
program that you installed. You already have the real form and just
need to find a way to get the program to display it for you.

You've heard it a few times from some of us, and even repeated it a
few times yourself. <g> I think it's just starting to sink in with the
OP, or at least I hope so. :-)

I don't think we'll ever know. He seems to have faded away with no real
sign that he understood the explanation. Good luck to him.
